What to prepare
- Adherence since the last visit — per medication, especially any you often miss.
- Why doses were missed — timing didn’t fit, worried about side effects, forgot.
- Changes you noticed — with dates.
- Supplements and other medications you take.
A record is supporting information; any change to your medication is decided with your doctor or pharmacist.
Keeping dose notes
One line is enough: "upset stomach", "dizzy", "felt fine". You can note only when something happens, but the date is what lets you answer "since when?".
Making the report in the app

Dose notes
On the Today list in Medication Reminder, tap the note icon next to a dose to attach a note to it.

Export a PDF report
On the Stats tab, choose a period and tap Export PDF report. After a short ad the PDF is created and ready to share. It includes the period, adherence, streak, and taken/scheduled and adherence per medication, for the profile currently selected.
A table? Use CSV
Export history (CSV) in Settings copies date, time, medication, status and note to the clipboard as a table you can paste into a spreadsheet.
